Payments Accepted

Payment in full and copayments are required at the time of your visit. Payment methods include personal or company check, Mastercard or VISA credit cards, and cash which is required for certain services. As a courtesy to our patients we will file for your benefits with your primary insurnace policy. Please note it is your responsibility to pay for all services recieved due to unmeet insurance deductibles or lack of benefits. The patient is responsible for obtaining their initial authorization from their insurnace carrier prior to their first visit. Medicare and Medicad insurance assignments are not accepted.

Return check and "no show cancelation" service fees of $30.00 are applied to the patient's account for each occurance.
